- Request Letter
- Copy of FIR including e-FIR/ police complaint/ court injunction order/ copy of plaint having the details of securities, folio number, distinctive number range and certificate numbers, if the market value of securities is more than Rs. 5,00,000
- Affidavit as per format prescribed by SEBI, duly executed and notarized
- Indemnity bond as per format prescribed by SEBI, duly executed and notarized
- Self-attested copy of ID and Address Proof of the holder
- Copy of advertisement regarding loss of securities in a widely circulated newspaper, if the market value of securities is more than Rs. 5,00,000
- Duly filled and signed ISR - 4
- Client Master List of the demat account of the holder
